High-voltage and battery-room access
Flags entry to HV zones, battery rooms, switch rooms and electrical rooms outside an authorised window.
Work starting before isolation

Risk & control objective
Electrical work is governed by procedure rather than by guarding: NFPA 70 and NEC in the US, PUWER and the electricity-at-work regime in the UK, and the isolation and permit-to-work systems every operator runs on top of them. The failure mode is almost always sequence — a step taken before the step that should have preceded it.
That makes the control objective procedural assurance rather than hazard detection. The Q3 chemical paper describes the target directly: an automated visual evidence pack per permit, from issue through closeout, with isolation-zone monitoring and LOTO verification support alongside it. What the operator gets is the ability to show that the sequence held, per permit, without asking anyone to reconstruct it afterwards.
