Risk & control objective

Stairway falls are high-frequency and low-attention: they happen on ordinary ground, during ordinary movement, and they rarely generate a report unless someone is hurt. Handrail use is the single behavioural control that correlates with the outcome, and it is visible in every stairwell camera a site already has.

The Q3 papers treat this as a behavioural-safety leading indicator rather than an incident category, and pair it with the physical condition of the stairway itself — blocked routes, damaged treads, missing guardrails. Both halves are observable continuously, which makes the rate the thing you manage rather than the injury count.
